Mitral Valve Prolapse with Primary Pulmonary Hypertention

Abstract
Since Barlow¢¥s.description in 1963, the midsystolicclick, either as an isolated physical finding or in association with the late systolic murmur, has been recognized as representing prolapse of mitral valve leaflet. The syndrome has been noted to be more frequent in female than in male, and associated with Marfan¢¥s syndrome, ostium secundum Atrial Septal Defect and atherosclerotic coronary arterial disease. But coexisting primary pulmonary hypertention has not been reported until recently.
We have recently had the opportunity to experience one case of mitral valve prolapsed associated with primary pulmonary hypertension.
The patient was 21-year-old female, who was younger sister of identical twin. She has exertional dyspnea, palpitation and polyarthralgia intermittently since 8 years ago.
Aside from above symptom, cardiac examination reveals late systolic murmur and accentuated pulmonic second sound. The cardiac catheterization demonstrated normal pulmonary wedge pressure and elevated pulmonary arterial pressure, and the cineangiograph showed the prolapse of posterior leaflet of mitral valve into the left atrium during the systolic phase.
We discussed generally the clinical feature including hemodynamic and angiocardiographic findings and reviewed pertinent literature published until recently.
